If you plan to participate in Shark Dive, please download
and print the Liability Waiver available via the link below.
You must complete, sign, and fax this waiver to Atlantis
Marine World at 631.208.0466 prior to your dive in order
to be able to participate.
If you have an existing medical condition, you must also
download and print the Medical Release available below.
Please complete and sign your portion, and have your physician
do the same. Once the form has been completed, it too must
be faxed to Atlantis Marine World at 631.208.0466 prior
to your dive.
